Exciting News Regarding Network Expansion!

               

We have exciting news to share with you!

As you may have heard, Kaiser Permanente has partnered with Unite Us to connect community-based organizations, health care providers, and public agencies in the Mid-Atlantic region. These efforts will expand the ServingTogether network and allow partners to serve a broader population, in addition to veterans and military families. This effort will bring in additional service providers to serve all community members in the Mid-Atlantic region, covering Northern VA, D.C., Suburban Maryland and Baltimore. The initiative is rolling out in a three-part phased approach that launches on June 23.

Communities across the country are connecting health and social services more seamlessly, and capturing service outcomes. Your team is a key part to this success — you’ve already been doing this with ServingTogether.

By expanding the breadth of the ServingTogether network, this results in an improved experience for all Unite Us partners; accessing the platform or how you use it doesn’t change, but the options for service delivery multiply for those you serve.

What does this mean for your organization? If you have additional programs that are not currently featured in ServingTogether, expanding beyond the military-connected community, the Unite Us team can help you add these programs to your organization’s profile in the platform.

 What’s staying the same?

  • Your login information.
  • Sending and receiving referrals from the Coordination Center, which will continue to act as a hub for veteran activity, monitoring military-related activities and performance.
  • ServingTogether’s focus on serving Veterans, Active Duty, Guard, Reserve, and their families.
  • Your relationship with ServingTogether’s dedicated team of Program Managers and Peer Navigators.

  • Please add any new program information to this Organization Update Form. To ensure participation by launch on June 23, please complete the form by June 9.
